Seriously Hooked

Has anyone knit EZ's Spiral Hat lately? I am using the instructions in The Opinionated Knitter and they seem simple enough. However, I keep ending up with 59 sts instead of 60 sts.

Could this be due to knitting on 2 circs? I certainly know how to "M1" both ways (the backwards loop on needle and lifting runner betw stitches and knitting into the back of it).

I had trouble making "Fetching" with two circs due to trying to make cables w/out a cable needle at the beginning of each needle - I simply couldn't "catch" the stitches there at all. I simply had to use a cable needle for those particular cables.

I wonder if something similar is going on here? Something mechanical that I'm simply not seeing?

Also, I don't understand why, but by the time I've knit two rounds of *P2, p2tog, k16, M1* I am not coming out even at all, even accounting for the missing 60th stitch! I understand the purls will spiral around but I'm more "off" than that accounts for.

Does this make sense? Can anyone help?

I have her books, love making hats, and will look into it. In fact my sister and I bought her first book together (split the price) since neither thought we could afford it.
However when one of us had it, the other wanted it. Of course we bought a second.

"Fetching" is a term I am not familiar with. ??

I don't have this pattern, but I have seen it, I think. In looking at *P2, p2tog, k16, M1*, that's 20 sts. If you are not knitting with stitch markers, would that help? It sounds like a p2tpg or m1 is being missed somewhere.
